INFORMED PARENTAL CONSENT FORM

Warmest Greetings!

We, the 4th-year student teachers assigned in grade 1 Our Lady of Pillar are
currently conducting a study entitled “ENHANCING THE VOCABULARY SKILLS OF
GRADE 1 STUDENTS THROUGH CROSSWORD PUZZLE” Crossword puzzle is a
word game that typically consists of a grid of squares, with each squares containing a
letter. Clues are provided to help the players solve the word vertically and horizontally.
In view of this, the researchers would like to request your permission to allow your child
to the said study. The participants will individually answer crossword exercises for 5
session that will take less than 10 minutes per day.
The potential benefits of the study might help educators to inculcate crossword
puzzles into the curriculum to improve the vocabulary skills especially in Filipino of
students. Rest assured that all information will be treated with utmost confidentiality.

Informed Consent Statement

GRADE 1 STUDENTS THROUGH CROSSWORD PUZZLE.” The study has been
explained to me and my questions answered to my satisfaction. I understand that my
child’s right to withdraw from participating or refuse to participate will be respected and
that his/her responses and identity will be kept confidential. I give this consent
