This checklist was created to serve as a general tool which can be used to
guide companies in creating a safe and healthy workplace. After reviewing

GENERAL SAFETY AND MISCELLANEOUS
Have you reviewed and are you meeting all OSHA requirements for your specific
industry and state?

Has a Job Hazard Analysis been completed?

EMERGENCY PREPAREDNESS AND FIRE PROTECTION


Are all employees familiar with your general and site-specific emergency plans?

HOUSEKEEPING
Are high-traffic and high-hazard areas free from debris and clutter?

PERSONAL PROTECTIVE EQUIPMENT POLICY


Is a Personal Protective Equipment policy readily available?

Has a Hazard Assessment been completed?

HAZARD COMMUNICATION PROGRAM


Is a program in place and training provided to employees?

BIOLOGICAL HAZARDS AND HAZARDOUS WASTE


IDo employees have a risk of being exposed to blood and other human bodily fluids? If
so, is a Bloodborne Pathogen Exposure Control Plan in place and training provided to
affected employees?

SHARP HAZARDS AND BROKEN GLASS
Do employees work with or have the potential to be exposed to sharps or broken
glass? If yes, have they been notified of specific handling and proper disposal
procedures?

FALL PROTECTION
Do employees have a risk of falling from elevated work surfaces (over 4 feet)? If so,
are appropriate fall protection systems in place?

Have employees been trained in fall protection requirements?

EQUIPMENT AND MACHINE SAFETY


Is there mechanical equipment or machinery located in the workplace? If yes, have
all employees been trained, and do the machines have the proper safeguards?

ERGONOMICS AND MATERIAL HANDLING


Do employees or vendors complete repetitive motions for at least 2 hours per day? If
yes, have those employees received an ergonomics assessment and been trained in
ergonomics?
