All teacher applicants must submit the following requirements to the School Screening Committee: a. Letter of Intent addressed to the Head of Office, or to the highest human resource officer designated by the Head of Office; b. Omnibus Sworn Statement c. Duly accomplished PDS (CSC Form 212, Revised 2017) with Work Experience Sheet (if applicable); d. Certified copy of scholastic/academic record such as but not limited to Transcript of Records (TOR) and Diploma, including completion of graduate and post graduate units/degree; ©. Photocopy of latest appointment, if applicable; f. Photocopy of Performance Rating in the last rating period(s) covering one (1) year performance in the current/latest position prior to the deadline of submission, if applicable; g. Certified copy of rating obtained in the LET/Board examination h. Certified copy of valid and updated PRC License/ID, if applicable; i. Certified copy of Specialized Training (i. NC, TM, etc.) j. Certified copy of Voter's ID, as proof of residency k. NBI Clearance 4. The teacher applicants are advised to register through the link https://bit.lv/SDOITEACHERAPPLICANT2024 upon submission of application to the School Screening Committee. All teacher applicants whether or not included in the recent RQA must undergo the assessment process and adhere to the requirements for application and assessment. 5. The Demonstration Teaching must be done in actual classroom set-up with learners in the grade level. The lesson must be taken from the prescribed learning competencies for the grade level and must be the assigned lesson on the day of the demonstration teaching based on the MELC/ Curriculum Guide. 6. The initial RQA shall include all the teacher applicants who have undergone the process of assessment regardless of points earned. There will be five initial RQAs to be submitted by the Division Sub-Committee such as Initial RQA for SPED, Kindergarten, Elementary, Junior High School, and Senior High School. The requirements (pertinent papers) of applicants shall also be forwarded to the Division Screening Committee for validation purposes.
