Madam:

I have the honor to apply for transfer from my present school assignment at Antipolo
Heights Integrated School, Pasacao District to Mandacanan Elementary School or nearby
schools in Libmanan North District, Libmanan, Camarines Sur.
My outmost desire for transfer entails the following reasons:
1. I have been teaching in the service for 4 years and 7 months at Antipolo Heights Integrated
School.
2. I am residing at Zone 5, Apad, Potot, Libmanan, Camarines Sur.
3. I am a nursing mother of my son, 1 year and 4 months old. (PD No. 603, Child and Youth
Welfare Code).
4. I experienced putting my life in risk. Antipolo Heights Integrated School is in a far-flung area,
where the means of transportation is by a motorized vehicle that during rainy season our
way home is risky due to slippery road and flood.
5. The delivery of my son was through cesarean section (CS) that is why my parents and my
husband are anxious and worried for my safety especially during rainy season.
6. To economized my weekly expenses for my fare in commuting from my residence to my
station and other expenses.
7. I want to pursue my masteral degree for professional growth.

I hope that I will merit your kind attention and consideration.
