FLY AVIUN CHARTER FLIGHT TICKET AND

HOTEL RESERVATIONS AND TOUR
CANCELLATION/RETURN FORM

In line with the Tbilisi - Managua charter flight ticket request of our customer, whose name
and surname is SAYDZHALOL AZIZOV, Flight dated 01.12.2023, cancellation/refund
procedures regarding the flight ticket and hotel reservations have been initiated.
The cancellation/refund amount of 11,000 USD will be refunded if the IBAN/VISA CARD
NUMBER is declared to the following e-mail address between 08.12.2023 - 12.12.2023.

• I would like to apologize to our valued passengers for the problem experienced due
to misunderstandings, and would like to state that I have included your cancellation
refund among the fees I paid between 08.12.2023 - 12.12.2023.
• We kindly request that it be sent as IBAN number (Name Surname, Iban number,
Swift Code).
• It must be sent with your VISA CARD NUMBER (advertising surname, 16 digits of
your card and expiration date).
• All official correspondence regarding cancellation and refund transactions are made
only via e-mail to iade@flyaviun.com. Ignore mail sent from different mailing
addresses.
