A male baby was Born on 4/4/2023 at 5-39 AM by Emergency LSCS (Foetal

Distress) at VMKVMCH, cried immediately after birth.

Meconium and urine passed after 24 hrs.
Mother’s LMP- 5/8/2022
EDD- 13/5/2023.
Baby’s birth weight was 2250 grams and was admitted in NICU for 4 days.
Baby was fed with Expressed breast milk on day 1 and day 2 of birth, and then the
baby was started on direct breast feeding & sucking good. No h/o Respiratory
distress, yellowish discoloration of eye, feed intolerance, blood in stools, fever ,
hypothermia, seizures , irritability , decreased activity.
AGE of mother :27 yrs
Parity: G1P1
Antenatal H/O - Uneventful, Booked & Immunised
Male / B.WT. 2.2KG / APGAR Score: 1’ - 7/10; 5’ - 8/10; Cried immediately after
birth.
VITALS:
HR: 146/Min TEMP:97.4F

RR: 44/min CRT:<2Sec

Spo2-98% @room air.

Head circumference: 31 cm, Length: 45 cm.

O/E: Cry, Activity good ,Normothermic.

S/E: CVS: s1 s2 + no murmur.

R/S: NVBS , NO added sounds.

P/A: soft ,no organomegaly.

CNS: normal tone ;moving all limbs equally..

Neonatal Reflexes present.
