Login into the

Select State and
Mobile Click on Card Delivery
ID Type
Application

Card Delivered Select beneficiary

Upload beneficiary’s
Authenticate the beneficiary
Click on Submit photo holding
(Face Auth or Finger Auth)

Enter the Code that was set
at the time of the One-Time
Login process.
• On the screen, the
user can see the
option of “Card
Download” and “Card
Delivery”.
• Click on “Card
Delivery”.
• Select State from the drop-down list and the ID Type as PMJAY ID
• Click on Get Details to view the list of the beneficiaries
• Click on Reset to search for the new beneficiary.
• After clicking on Get Details, the list of
beneficiaries associated with that particular
PM-JAY ID will open
• Select the beneficiary from the list.
• Click on Self to authenticate the
beneficiary
• Click on Family to select the family
members to deliver their Ayushman
Card
• After selecting “Self” and/or
“Family”, the Aadhar number of the
beneficiary selected for “Self” will
reflect.
• Select “Face” or “Finger Auth” for
initiating the Aadhaar authentication
process.
• Once the beneficiary is selected for
authentication, their Aadhar number will be
auto-fetched.
• On successful “Face Auth” you will
get the message “Images captured
successfully” processing…
• After selecting “Face Auth” for the Aadhaar
authentication process, the camera will open
to capture the beneficiary’s face.
• On unsuccessful Aadhaar Authentication,
you will get the message “Info (730):
Capture Failed. Please Improve Lighting and
Try again”.
• On an unsuccessful “Face Auth”, try the
following:
 Face toward the light source
 Move closer to the camera
 Clean camera lens
 Hold still and blink
• After successful “Face Auth” you will see the
highlighted button saying “Authentication
Successful”
• Now click on “Take Photo”. Capture the
image of the beneficiary which will reflect on
the PV card.
• Now you need to accept the “Terms &
Conditions” and click on “Deliver”.
• On successful submission, a message
will appear: “The proof of Ayushman
Card delivery has been successfully
submitted for record and audit
purposes”.
