That I, JOHN MARK T. ULIT , of legal age, single, Filipino with resident
address at BLOCK 9 LOT 7 LEGIAN 2 NORTH CARSADANG BAGO, IMUS CAVITE
after having been duly sworn to in accordance with law hereby, depose and state that:

That I am a applicant for New Certificate of Public Convenience to operate a TNVS Service on
the route: Metro Manila to any drop-off points in Luzon and vice versa with the use of ONE (1) unit with
Case No. TNVSMH05290005;

That I am the registered owner of the subject unit in my application;

That I am legally qualified to operate said unit as evidenced by my Birth Certificate with Local
Civil Registry No. 7181, Driver’s License with No. D06-17-013594;
That there is a public necessity for TNVS service on the route applied for owning to many
commuters on the said route;
That I am executing this affidavit to attest to the truth of the foregoing instrument further for
whatever legal intents this may serve best.
