Safety Management Plan Pag 27 Qetacec 0 - INTRODUCTION 0.1. scope, The current Security and H Ith Management Plan, hereafter called SMP, applies to the execution of the Work Assembly Supervision of 30kV Indoor Switchgear and unloading transformers in the company COCA-COLA SABCO (MOGAMBIQUE) S.A.R.L located in Maputo, Mozambique. This document was elaborated to gather all of the relevant information and indications to implement workers’ security and health preventive measures, thus fulfilling the iegal requirenents related to such matters. Being a cynamic document it is presented as a dossier to facilitate updates and queries. The SMP should remain in the shipyard during the entire construction phase, accessible to appointed responsible persons, and the Executing Entity is responsinle for maintaining and updating the documentation. Copies should be provided to use the documents in other sites. The Current SMP 1 be technically validated by the Work's Safety Coordinator, and approved by the Employer. The SMP is intended as a disclosure of the obligations, methods and procedures to be published to increase work safety for each and every intervening party, and to secure better Productivity and Quality levels. The objectives of the current document are as follow: Y To identify and assess the risks associated to the construction process and the materials used during the works; ¥ To guarantee that the Executing Entity, as well as the involved workers, know the risks they will have to consider to control and minimize occupational accidents, and to implement security management throughout the organization; Y To guarantee the execution of all works in proper security and health conditions; To secure good productivity by implementing good work conditions; To reduce labour accidents and the associated social and economie costa; To perform the works with the specified quality, in organized and environmentally acceptable spaces; Y To guarantee the disclosure of the security rules contained herein to all work administration levels; Y To comply with OHS legal requirement. ‘Satety Management Plan Pag a7 Qefacec 0.2. HEALTH AND SAFETY POLICY EFACEC considers Health and Safety as fundamental in all its activities. Thus, the Health and Safety Policy shall be understood as a pricrity by all Collaborators and followed in their daily conduct, regarding the continuous improvement of project performance. Health and Safety Policy Objectives: (a) The avoidance of risks, e.g. where appropriate by selecting non-fragile roofing materials, or by avoiding the disturbance of contaminated land where practicable; (b) The evaluation of unavoidable risks, i.e, evaluation the likely consequences of such risks: (©) The combating of risks at source. If risks cannot be avoided, they should be combated at source as far as is practicable, e.g. by specifying brush application of a treatment rather than spraying to reduce solvent exposure, or by specifying finishing materials containing less harmful constituents; (@) The adaptation of work to the individual, especially as regards the design of places of work, the choice of work equipment and the choice of systems of work; (e) The adaptation of the place of work to technical progress. Taking advantage of technological progress often offers opportunities for safe and more efficient working methods; (2) The replacement of dangerous articles, substances or systems of work by no. dangerous or less dangerous art les, substances or systems of work, e.g. replacing solvent based paints with water based ones where practicable; (g) The development of an adequate prevention policy in relation to safety, health and welfare at work, which takes account of technology, organization ef work, working conditions, social factors and the influence of factors related to the working environment. The Health and Safety Plan for the site should act as the focus for bringing together and coordinating the prevention policies of the various contractors involved in the project; (b) The giving of priority to collective protective measures over individual Protective measures. RISK ASSESSMENT AND HIERARCHIZATION Professional sk assessment is a dynamic process designed to evaluate the magnitude of workers’ health and safety risk at the workplace, given the circumstances the hazard may occur, to obtain the necessary information for adopting preventive measures to minimize the eccurrence of accidents. By a detailed description of the works the hazards were identified, the risks were appraised and quantified, and finally the preventive measures were defined. The risk analysis made for the current work seeks to identify the risks inherent to both the shipyard and the work, since its inception and development until completion. This is intended to be the most inclusive possible, not ignoring safety-related factors such as raw materials and equipment, and installation/work methods to use This method allows one to analyze and rationally List all aspects directly or indirectly related with safety, identifying the werent risks and proposing efficient functional solutions at the smallest cost. For each work operation a Risk Assessment is made, and at the same tine this document sets forth the prevention measures, the Inspection and Prevention Procedures (IPP) and the Safety Instructions applicable to each situation found. This is a reference document that mirrors our concerns related to safety, and the consequent prevention of occupational accidents Risk Assessments are found in attachment 1 - “Risk Assessment” 3.SITE PROJECT 3.2. SITE A shipyard is the place where con to the contract occur, as well as the places where their direct support activities are performed. Thus, included he shipyard are those worksites located off the central shipyard’s limits. The work support shipyard will centralize all of the coordination, organization and work preparation operations. Therefore it will and equipment to be installed, as well as the parking 1 be the storage site for all materials for equipment and assenply tools, machines and vehicles. Its location will be studied and strategically placed within the work area, considering the worksite development and accesses, to optimize the logistics support. EQUIPMENT USE AND CONTROL The equipment to be used in the work, besides those of contractors and independent workers, will have their own list available at the shipyard. The contractor must guarantee that the equipment is in good use conditions, and that the necessary control documentation is on hand before their operation at the worksite, A file must be organized with the following documentation, related to each piece of equipment The periodic technical verification records for all the equipment at work, and the static and dynamic test records for the cranes, if any. New machines acquired after January 1995 should have: + CE statement (certificate of compliance) + Instructions Manual + ce marks The mobile equipment and the machinery used for raising and moving loads acquired before January 1995 should comply with minimum security requirements legal requirements - DL 50/2005); Evidences of technical skills (machine conductors or drivers should have appropriate training) and medical aptitude records for the personnel, Civil Liability insurance, as applicable, Documents of transport trucks must include: Booklet and Property Title Record; Vehicle insurance; s) Periodic Inspection Record (if the truck age so requis The equipment must also have operational the reverse gear sound alarm and the intermittent lights, as applicable. The shipyard will have appropriate and duly identified containers to (temporarily) storage the produced waste, separated per type. 3.5. EXISTING CONDITIONS The conditions record is intended to detect and document the existence of circumstances at the worksite and/or its surrounding areas that may become restrictions to the regular execution of the works. WORKERS IDENTIFICATION EFACEC is responsible for identifying all the workers on the worksite, including those of contractors, subcontractors and independent workers, if any, whe work at the worksite for more than 24 hours. EFACEC must also make sure that all workers received the Individual Protection Equipment (IPE) for permanent use, and that they were trained for the risks that said equipment seek to protect them against. For that, an IPE distribution record must be kept, signed by each worker, For foreign nationality workers, the work visa or residence authorization and the respective validity and labour agreement should also exiet and be provided upon request The entire documentation of the personnel at the worksite should be actachnent 3. Safety Management Plan Pag 317 a) Collective Protection Plan To reduce occupational risks, the necessary collective protection measures will be applied, which will have priority over the individual protection measures The Collective Protection Plan applicable to this work defines the protection equipment to use and their respective implementation at the appropriate «: according to the risks the totality or groups of workers may be exposed to (fall from high places, fali of objects, etc.) Regardless of any other equipment that is necessary to perform the works, one should consider: © Emergency lighting; © Security signs; © Barriers and delimita © Guard rails; © Fire extinguishers; © Work platforms. b) Individual Protection Plan Individual Protection Equipment (IPE) is any equipment, as well as any complement or accessory, to be used by the worker for protection against risks, thus preserving his/her safety and health. They are intended to mitigate the risks associated to the take each worker carries out. IPE should be used whenever the existing risks cannot be avoided in a satisfactory way by technical collective protection means, or by work organization measures, methods or processes. IFEs should also be used as complementary preventive measures whenever deemed appropriate. To define the IPE that each worker should use, one must distinguish them between permanent and temporary. The former are intended for use by any worker while at the shipyard (for instance, helmets and boots with steel tips). The latter are used by the worker according to the type of task (for instance, protective earplugs in environments with high-intensity sound levels), and extraordinary work conditions ne may be subject to (harness while working in high levels, either when collective protection measures cannot be adopted, or to complement them as accessory measures) The use conditions for the Individual Protection Equipment will be determined according to the risk severity, the risk exposure frequency, the characteristics of ‘Saoty Management Plan Pag 127 Qetacec each worker's workstation, and the equipment’s behaviour. A protection equipment set will be defined for each specific work. The contractor is responsible for the Individual Protection Equipment distribution control records, even if a subcontracting chain exist The employer must provide the Individual Protection Equipment, and make sure that all instructions required for the correct use of the equipment are tranamitted and inform their workers on the risks each one is designed prevent. Thereupon the worker must give proof that he received the IPE’s and was instructed about his obligations, by signing the IPE distribution record. Any anomalies or defects found in should also be reported. 4.5. WORKER'S HEALTH PLAN The workers' Health Plan, is intended to make the Employer Entity liable for its workers’ health, by verifying their physical and psychic fitness for the fulfilment of their roles, and the consequences of the work and its health conditions. Thus the EFACEC, contractors and independent workers are responsible for conducting medical examinations on all vorkers at the worksite, For that purpose, EFACEC will provide at the support shipyard the workers’ medical records, including contractors and independent workers. Each vorker’s individual record should contain the date of the lest medical examination, and the result of the medical exam duly signed by the Company's physician attesting the worker's aptitude. The aptitude records for collaborators aged between 1 and 50 are valid for 2 years. The remaining collaborators will take exams annually. Workers involved in accidents that result in temporary incapacity for a period in excess of 30 days must, before returning to work, be submitted to medical examination. 4.6. FIRST AID EFACEC is responsible to provide the worksites with a sufficient number of peopled aid to trained as First-aider. They must have appropriate training to provide Fir eventual accident victims and have immediate communication means to answer calls and contact the necessary help units in each emergency situation. pAACEC will assign the workers qualified for acting as First-aiders. First Aid will be provided according to the “Mmergency Response Instructions” found in the Emergency Plan filed in attachment 4 - “mergency”. The Emergency Plan also contains 4 table with the emexgency contacts Sefety Management Plan Pog 1917 Qetacec 4.7, FIREARMS AND AMMUNITION The possession of any type of firearms or anmunition at workplace is not permitted at any Contractor or Subcontractor employee. ‘The information relative to this question will be transmitted to workers through Notice Boards near the most frequented areas. Those Notices will inform of the Prohibition of the Possession of firearms or anmuniticns in the Workplace. Concerning training of the workers, these are informed during the Induction training and on “Toctbox Talks” done periodically. Concerning monitoring, the possession of fire: © or ammuniticns is controlled by ally and alienator ransacks. 5. CONDITIONS FOR SELECTING CONTRACTORS, INDEPENDENT WORKERS, AND WORK ‘EQUIPMENT 5.1. CONDITIONS FOR SELECTING CONTRACTORS AND INDEPENDENT WORKERS The contractors and independent workers will enly be auth: zed to begin their Work activities after delivering at the Shipyard all the documentation listed below: Y Permits ¥ Work Schedule; ¥ Statement of non-indebtedness to Finances; ¥ Statement of Non-Indebtedness to Social Security; Y Copy of the Taxpayer number; Y Occupational accident insurance policies for all workers and independent workers hired by you, as well as the corresponding receipts; vcs ability insurance policies and the respective premium payment receipte; Y Documentation related to the work equipment, according to item 3.3. For the workers and independent workers hired by you who work at the site for more than twenty-four hours, a Personnel List must be provided with the following information: a) The complete identification, and the residential address; b) The taxpayer number; ©) The y beneficiary nunber; d) The professional category; e) The forecasted beginning and end dates for the works at the sites VY Health Plan for the workers and independent workers hired by yous V The medical aptitude records for your personnel. ‘Salety Management Plan Pag tan7 Qetacec EFACEC will file the documentation listed above at the Shipyard. The services rendered by contractors and independent workers also rely on the full compliance with the safety instructions determined by EFACEC, as well as the regulations in effect. con actors cannot subcontract any works agreed upon with the General Contractor within their scope without his approval, and all contractors admitted to the current work must have authorization by the Work Inspector and/or Employer. EFACEC must identify, by the Sth of each month, the Contractors already selected. The work equipment and the respective documentation will be submitted to a check before conmissioning, to attest their proper operation conditions and compliance with the safety and environmental best practices 6. INFORMATION AND TRAINING SYSTEM FOR ALL WORKERS 6.1. Whenever necessary, meetings will be held with the workers to discuss occupational risk prevention issues. These meetings may be convened by any worker, by the Project Manager or EFACEC’s Safety Technician. 6.2. TRAINING SYSTEM FOR ALL WORKERS EFACEC must conduct training/awareness actions for the workers involved in the work. These actions should be held: At the beginning of the work, and whenever new teams start to work; At the beginning of critical safety activities) Whenever new work methodologies are introduced; CRS Whenever recurring incident situations are noticed. In generic terms, the information conveyed to the workers will follow the assumptions below: Y Knowledge, by each collaborator, of the risks he is exposed to; Y Knowledge, by each collaborator, of the prevention measures to be adopted at his workplace) Y Knowledge, by each collaborator, of the emergency and help measures to be followed in case of accident. The information and awareness actions must be planned so that they precede the risk- posing activity, regardless of the reinforcement actions made during the works. ‘Thus, three different types of actions are foreseen, according to the timeframe and operation to perform; Reception actions - of short duration, covering topics such as disclosure of the work emergency plan, general risks of the work, mandatory use of appropriate IPE during the work activities, etc. This reception action must be provided before the worker begins to perform any type of work activity. Awareness actions - also of short duration, intended for the difserent groups of workers to convey the work procedures developed for activities posing special risks, or other conditions whose information is required. EMERGENCY PROCEDURES This chapter defines the procedures to be followed during an emergency situation at the worksite. Therefore is must be widely known, and have its contents disclosed in training sessions. An emergency plan is a mechanism that determines 2 hierarchical and functional structure for the different inter: jal and external agents called to intervene on che identified risk sites. Tt establishes @ coordination system for both the company’s and external huran and material resources An emergency is an unexpected event that may cause death or sever injury to cellaborators, customers or the public in general, as well as significant environmental impacts, thus compromising the public image of the Company. Attachment 4 - “Emergency” contains the Emergency Plan applicable to the worksite. 7.1. EMERGENCY MANAGEMENT a) Emergency procedures All personnel has the overall duty to contribute for the detection and reporting of emergency situations, to trigger the intervention mechanisms appropriate for the situation. one showld always nave mind that the information provided must not be exaggerated, but realistic and concise.
