The undersigned, Cardo Dalisay, accuses Juan Dela Cruz of homicide, committed
as follows, to wit:

On or about January 10, 2024, at about 10:00 PM in the evening, at Barangay San
Roque, Mabini, Davao de Oro, Philippines, the said accused willfully, unlawfully,
feloniously, and by means of a knife, committed homicide upon the undersigned’s son,
Pedro Dalisay, directly by overt acts to wit: stabbed Pedro Dalisay multiple times on the
chest with a knife, causing severe injuries that resulted in death.

The undersigned executed this affidavit to attest the truthfulness of the foregoing
facts and to support the filing of Criminal Cases against
Juan Dela Cruz for violations of Article 249 of the Revised Penal Code of the Philippines.
