That on July 25, 2023 a request with the Land Registration Authority (LRA) of the
following were made: (a) Deed of Conveyance, (b) Transfer Tax, (c) BIR eCAR, (d)
Secretary’s Certificate, and (e) Other pertinent documents related thereto;

2. That a certification was issued to me stating that “all documents, relative to CCT No. N-
8862 leading to the issuance of CCT No. (N-11231)004-2023008942 are among those
documents that cannot be located in the Registry inspite of diligent search and efforts to
locate the same”

3. I am executing this Affidavit to attest to the truth of the foregoing statements and to
request the issuance of the aforementioned documents namely: a) Deed of Conveyance,
(b) Transfer Tax, (c) BIR eCAR, (d) Secretary’s Certificate, and (e) Other pertinent
documents related thereto in lieu of the one which was lost.
