I, ROWELL QUINTAS RODRIGUEZ, of legal age, Filipino, married, and a resident of

Barangay Sta. Monica, Puerto Princesa City, Palawan after having been duly sworn to in
accordance with the law, hereby depose and say:

1. That I am a citizen of the Republic of the Philippines;

2. That I was born on September 24, 1964 in Puerto Princesa City, Palawan;

3. That my parents are both Filipinos namely Rodrigo B. Rodriguez and Julie Q. Rodriguez;

4. That my wife, Janet Baby P. Venturillo is also a Fililipino citizen; and

5. That I am executing this affidavit, to attest to the truthfulness of the foregoing facts and for all
legal intents and purposes.
