Note
Positive Gram stain report indicates type of microorganism, their morphology and specimen based on the
type of cells seen.
Negative Gram stain report indicates the absence of microorganisms/ cells. False negative may be due pauci
bacillary nature of specimen or certain organisms like (Treponema, Mycoplasma, Chlamydia, and Rickettsia)
do not take stain as they lack cell wall or are too small. An- aerobic bacteria, Leigonella, Brucella,Francisella,
older culture and bacteria exhibiting effects of antibiotics will be difficult to interpret.
The test cannot be used to speciate the organism.

Comments
The Gram stain is a rapid stain used extensively in microbiology for the preliminary differentiation of
microbiological organisms directly from clinical specimens or positive culture growth. The Gram stain can be
a reliable indicator to guide initial antibiotic therapy in diverse bacterial infections.
This stain is used to identify the presence of microorganisms in normally sterile body fluids (cerebrospinal
fluid, synovial fluid, pleural fluid, peritoneal fluid). It is also used to screen sputum specimens to establish
acceptability for bacterial culture (<25 squamous epithelial cells per field is considered an acceptable

Note
Positive AFB smear results provide a first indication of mycobacterial infection and potential TB disease.
Smear-positivity and grade indicates relative bacterial burden and correlates with disease presentation.
Patient therapy may be initiated for TB based on smear result and clinical presentation-Changes in smear
status important for monitoring response to therapy.
Does not distinguish between viable and dead organisms nor differentiates species of Mycobacteria.
A negative AFB smear may mean that no infection is present, that symptoms are caused by something
other than mycobacteria, or that the mycobacteria were not present in sufficient numbers to be seen under
the microscope.

Comments
Mycobacterium tuberculosis is a major health problem in India accounting for 25% of global TB burden. Due
to the airborne spread of the disease have wide implications on public health making rapid diagnosis and
treatment of the infected patient essential. Nontuberculous mycobacteria infections also cause significant
morbidity and mortality in humans, particularly in immunocompromised persons. Detection of acid-fast bacilli
