to join/participate in the Homeroom Gardening and Beautification Activity on June 28, 2023, from 1:00 PM to 5:00 PM.

I hereby give consent to use his/her personal time in Grade 10 - Bantiling advocacy in Gardening and Beautification
of the school and that the school shall not be liable for the untoward incident that may happen.

I further consent and understand that the time and effort that will be spent in the said activity will be beneficial not
just for the school but to my daughter/son as well.
