AFFIDAVIT OF SOLO PARENT

I, LESLIE JOAN CORTEZ LIM, of legal age, Filipino, presently residing at B20 L18
Green Forbes Loma de Gato, Marilao Bulacan, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and say that:

1. I am applying for registration as a Solo Parent before the Social Welfare and
Development Office in order to avail of the benefits provided by law for the same;
2. I have one minor child namely, Sean Rainne Lim, who has been living with me in the
same residence;
3. I am the biological mother and of my child and was never married;
4. For several years, I have been living alone without the help and assistance of the
biological father of my child;
5. I have the sole custody and parental authority over my child; and
6. I am executing this Affidavit to attest to the truthfulness of the foregoing facts and
for purposes of complying with the requirements for the application as a solo parent
before the Social Welfare and Development Office.
