ISLAMIC RELIEF WORLDWIDE Afghanistan Country Office We are dedicated to alleviating the poverty and sufferings of the world’s poorest people SHORT TERM CONTRACT CONTRACT NUMBER: STA-IRW AGREEMENT between the ISLAMIC RELIEF WORLDWIDE (House No. 17, Street No 5, Qala- Kabul Afghanistan), hereinafter referred to as "IRW” and” Mohammad Farid Rahmani hereinafter referred to as "the Short-Term Appointee (STA)." WHEREAS, IR shall employ the employee and the Employee shall serve IR, as MEAL Assistant. The employee will work under the direct supervision of the MEAL Officer with such duties and responsibilities as stated in the Job Description attached and other duties as may be assigned to the employee by the supervisor. NOW, THEREFORE, the parties hereto agree as follows: 1, STATEMENT OF WORK: The duration of STA services will be till 31% March 2024. The Employee whilst working in IR shall abide by Child Protection Policy & Employee Code of Conduct and shall not engage in activities outside the scope of his/her employment. 2. PERIOD OF PARTICIPATION. STA’s tenure 2s MEAL Assistant shall commence from 24 December 2023 and shall be completed on 31" March 2024 unless this contract is terminated earlier in accordance with its terms and conditions. 3. PROBATIONARY PERIOD: One Month 4. Duty Station: Takhar 5. COMPENSATION: For the services rendered to IR, STA shall be paid AFN 24,704 under UNDP/ABADEI Project (AS-8-Step1) per month. 6. Medical Allowance: 4,000 AFN 7. Transportation Allowance: 3,000 AFN TERMINATION: 8. Either party may terminate this agreement with 15 days’ written notice. However, the notice period during the probationary is only 7 days. This agreement can also be terminated with immediate effect ifIR’s project funding is terminated for any reason. 9, IFeither party terminates this agreement without proper notice of 30 days; that party will have to pay one month's salary to the other party. 20. In case the contract is terminated by the employee, she will be required to complete a handing over document and an exit form. In case the employee fails to follow the procedure as mentioned above, he/she will not be entitled to any claim against Islamic Relief Afghanistan. Islamic Relief shall have a right to withhold the last salary of the employee plus any other existing benefits/pending dues in absence of Proper Handing over documents. However, IR reserves the right to take necessary action to recover any assets or resources in custody of the said employee. 12. An employee leaving the organization must ensure that any dues against Islamic Relief are settled in full DISPUTE RESOLUTION A. Dispute Resolution: All disputes arising from this contract shall be handled by friendly negotiations and through local and accepted mechanisms of mediation. Should negotiations and mediation not result in resolving the dispute, the issue shall be presented to a three panel arbitration counsel selected by IR. The prevailing law shall be the law of Afghanistan. B, Language of the Agreement: In the event of a dispute, the prevailing version of this employment agreement shall be the English version. 11. SICK LEAVE: the employee may be granted personal sickness. Sick Leave is accumulated at One day per month worked. Any employee who is sick for three consecutive days must bring a certified note from the attending physician 14, Annual Leave: The employee is eligible for 2 working days annual leave per month. Annual leave should be planned and approved in advance. Prior written approval from the employee’s supervisor must be obtained to use annual leave which should be submitted with a plan regarding the scheduling of activities for which the employee is responsible during the employee's leave. All annual leave must be taken within the period of this contract and no unused annual leave will be turned to cash. The annual leave will not be granted during the probationary period. Safeguarding: 1, The staff should always adhere with safeguarding as well code of conduct pursuant to the terms of this agreement. 2, The employee shall ensure that all allegations, suspicions and incidents of abuse are reported immediately to safeguarding focal point/CRM and shall promptly take appropriate disciplinary action against any employees who is engaged or is alleged to be engaged in any of the improper conduct. IRA started its humanitarian activities for Afghan Refugees in Pakistan and Afghan people inside since then; IRA continued assisting in emergency relief, food security, women empowerment, git! education, orphan sponsorship, health, emergency education, shelter, Ramadan, and Qurbani programs. Main Duties & Responsibilities: * Developing operational monitoring tools (checklist, questionnaires, databases, interview and survey forms, etc.) * Develop weekly, monthly monitoring and evaluation plans on lifesaving health services to target communities in the province, outlining data collection, analysis and reporting tools and processes. + Perform field visits to the target areas to assess the results of the project activities * Develop comprehensive monitoring and evaluation reports outlining project achievements, weaknesses and recommendations and highlighting corrective actions to be taken to tackle weaknesses Recommend tools and strategies to increase program performance and results. * Collects, analyze. enters, and reports Projects data and information accurately, thoroughly and in a timely manner. * Responsible to ensure the quantity and quality of lifesaving health services project Activities. © Conducts monitoring visits to ensure quality implementation of project at selected target areas, Page | of 2 Overlook activities of team selecting beneficiaries’ selection committee to select vulnerable beneficiaries based on the criteria Conducting Post Implementation Monitoring to.
