We, the researchers from the grade 12 HUMSS, Section Uranus, would kindly ask your
permission to conduct a survey to the Grade 12 HUMSS students of Seguinon Stand Alone
Senior High School, for our study entitled “Factors Affecting Low Academic Performance of
Learners”, which aims to provide useful insights and intervention in having a low academic
performance of the student in Seguinon Stand Alone Senior High School. We ensure, Sir, that
the data gathered in our survey will be kept confidential and would be utilized for academic
purpose only. Therefore, we would be grateful if you would grant us permission to conduct
our survey with Seguinon Stand Alone Senior High School students, especially Grade 12
