This is to certify that CORALYN A. YLAGAN was Employed last January 20, 2019 up to January
28, 2021 and worked as Private Caregiver to Consuelo Cirujano 69 years old who has a Breast
Cancer and Bedridden she rendered eight (8) hours per day Monday to Friday.

During this period her services were found to be satisfactory in carrying out the job duties, her
responsibilities were to:

 Administer medications, treatment plus other caregiver services according to physician

instructions conditions of the patient.
 Direct patient in good health habits.
 Give information to family in treatment of patient maintenance of healthful
invironment.
 Monitoring of the blood pressure of the patient.
 Changing diaper
 Lifting patient left to right side vice versa
