ROOT CAUSE ANALYSIS REPORT OF CHILLER NO 5

BREAKDOWN
BREAKDOWN PROBLEM: - There was a motor trip alarm in VFD
display of COMPRESSOR that “MOTOR SHORT CIRCUIT”.

1st WHY-: We found the cause of problem was due to moisture in

the motor terminal.

The moisture in the terminal was leading to electrical conductivity

problem that’s why it was motor was getting tripped by showing
“MOTOR SHORT CIRCUIT”.

2nd WHY -: The air entered was form the bolt fitting side of
terminal box and due to refrigerant flowing below the terminal
resulting that air got condensed and become moisture.

3rd WHY -: The bolt of terminal box got lose so the air made his
way to enter from this area.

4th WHY -: The OEM recommendation of frequency for checking all

tightness of electrical connection and bolts was not followed.

SOLUTION -: The frequency of checking all tightness of electrical

connections and terminal bolts should be followed as per OEM
recommendation and also, air tight or weatherproof enclosures
should be provided so that the air (moisture) should not enter in
terminal box.
