SBA COMMITMENT FORM

Instructions:
1. To be completed by students who are taking classes outside of Wolmer’s Boys’ School but
who will be sitting the exam at the Wolmer’s Boys’ Centre. NOT to be completed by the
students enrolled in WBS Extension Institute or the Accelerated 3rd Form students doing
Mathematics.
2. The teacher supervising the SBA MUST be employed to Wolmer’s Boys’ School.
3. RETURN the form attached to the CSEC or CAPE Examination Entry Form to the Bursar’s Office
between Wednesday, 15th, and Friday, 17th November 2023.
4. PLEASE COMPLETE THE FORM IN BLOCK LETTER.
