The order of set of required documents for

STUDY VISA
 Valid passport;
 Completed through the Internet and signed application form with
color photo (35 mm x 45 mm, white background) attached to
appropriate place;
 Photocopy of passport (identification page and all pages with
valid visas);
 Invitation for Study (issued by Ukrainian State Centre for
International Education);
 Valid health insurance (signed and stamped);
 Personal bank statement or Affidavit and bank statement of
sponsor (as a Proof of the sufficient financial means for the intended period
of stay and return to the country of origin);
 Ticket reservation to Ukraine;
 Photocopy of Invitation for Study (front and back side);
 Birth certificate*;
 Educational documents* with information about previous
education;
 Health certificate*;
 Policy clearance certificate*;
 Payment slip for consular fee.
Supporting documents
 Photocopy of Health insurance;
 Photocopy of Birth certificate;
 Photocopy of Police clearance certificate;
 Photocopy of Educational documents
