1. No standard clerking sheet for MCH unit CME ATUCU/ ATUCU or
leads to poor documentation and incomplete refreshment course
records. To implement proper documentation
using ATUCU 5 clerking form

1. All drugs are clearly documented timing
and dose
2. Parents are counselling on the dose and
frequency of the medication
3. Antibiotic (C-Penicillin, Gentamicin,
ampicillin) are available in ETD as floor stock
