INSTRUCTIONS

1. Please deposit Rs. 3100/-in any HBL and attach Bank Challan No. 4.
2. In case of regular student, Form and Photograph will be got attested by the
concerned Head of Institution where he/she was studying. In case of private
student, Form and Photograph will be got attested by any Head of the
recognized Institution within the jurisdiction of BISE, Rawalpindi.
3. An affidavit attested by Notary Public or Oath Commissioner has to be
attached.
4. A copy of certificate has to be attached.
