LIABILITY WAIVER

I, _______________________________________________, acknowledge that I am voluntarily

entering the premises of the Pangasinan State University (PSU) Bayambang Campus. I understand and
accept that by entering the campus, I am doing so entirely at my own risk.

I understand that PSU has taken reasonable measures to provide a safe environment for students,
faculty, staff, and visitors. However, I acknowledge that there are inherent risks associated with being on a
university campus, including but not limited to accidents, injuries, and exposure to contagious diseases.

I agree to indemnify and hold harmless PSU, its officers, employees, agents, representatives, and
affiliates from any and all claims, liabilities, damages, costs, and expenses, including attorney fees, that
may arise directly or indirectly from my presence on the PSU Bayambang Campus.

I certify that I have read this liability waiver and fully understand its contents. I voluntarily agree
to its terms and conditions and acknowledge that my entry onto the PSU Bayambang Campus is entirely
at my own risk.
