PHOTOGRAPHY CONSENT FORM / RELEASE

permission to 220 Services L.L.C. of Michigan representatives, to take
and use: photographs and/or digital images of me for use in portfolio
publications. This may include printed or electronic publications
representing “220 Photo”, “220 Services”, or “LevelUp Images”. This
does not include any print publication or website under another
photography persona. I further agree that my name or alias may be
revealed in descriptive text or commentary in connection with the
images. This includes but is not limited to Model Mayhem “credits,
Instagram or other social media platform “tags” and related mediums
so long it represents me a positive manner. I authorize the use of
these images without compensation to me. All negatives, prints,
digital reproductions shall be the property of 220 services L.L.C. .

portfolio publications. This may include printed or electronic
publications representing “220 Photo”, 220 Services” or “LevelUp
Images”. This does not include any print publication or website under
another photography persona. I further agree that my name or alias
may be revealed in descriptive text or commentary in connection with
the image(s). This includes but is not limited to Model Mayhem
“credits”, Instagram or other social media platform “tags” and related

photographs and/or digital images of said client at their digression.
This may include any print or electronic medium.

I also provide this client with the right to tag “220 Photo” or “LevelUP
Images in the images, until which point, if any, I decide otherwise.).
This includes but is not limited to Model Mayhem “credits, Instagram
or other social media platform “tags” and related mediums so long it
represents me a positive manner. I authorize the use of these images
without compensation to me, under the condition that all negatives,
prints, digital reproductions shall remain the property of 220 Services
L.L.C. and will not be sold or distributed as the works of any other
photographer or business.
