Patient awake,conscious, not in distress, well oriented
Rashes on hand and leg +, not in trunk, small papules not itching in character
Hermann’s sign + (rash , petechial)
Abdominal tenderness epigastric region +
No liver enlargement
No lymphadenopathy
No gum bleeding
No signs of hemorrhage
No seizures
Signs of polyuria
Clear heart sounds
Body malaise+
IMPRESSION:
Dengue fever without warning signs
SUMMARY:
A 29 yr old women,bank employee presented with 5 days history of abdominal pain
associated with on and off fever, severe body malaise, rashes on hand and legs, shortness of
breath, headache and nausea. No alteration in mental status, she was well oriented,
suggestive of dengue fever without warning sign
DISCUSSION :
SALIENT FEATURES:
29/ female, bank employee
Occasional drinker and non smoker
Epigastric pain
Shortness of breath
Rashes on hand and legs
Hermann’s sign + (rash , petechial
Fever on and off, nausea and headache
Severe body malaise
No co-morbidity
Previous history of dengue in 2013, hospitalized
Normal vitals
Polyuria
Dengue fever
Dengue (break-bone fever) is a viral infection that spreads from mosquitoes to people. It is
more common in tropical and subtropical climates.
Most people who get dengue won’t have symptoms. But for those that do, the most
common symptoms are high fever, headache, body aches, nausea and rash.
Symptoms occur, they usually begin 4–10 days after infection and last for 2–7 days
Common sympotoms of dengue :
severe abdominal pain
persistent vomiting
rapid breathing
bleeding gums or nose
fatigue
restlessness
blood in vomit or stool
being very thirsty
pale and cold skin
feeling weak.
