CAUS! Attachment-1 provides two types of C&E format which shall be standardized and utilized for Cause & Effect Diagram to be developed for all process units. ‘Type A is standard format without PERMISSIVE CONDITION section. ‘Type B is standard format with PERMISSIVE CONDITION section. ‘When permissive conditions have to be considered, Type B shall be applied. (Type A shall be used as much as possible.) ‘And A4 and A3 size formats are prepared for both types of format. Suitable size format can be applied accordingly. (A4 size shall be used as much as possible.) Additionally, dedicated forrhats for F&G system are prepared. For F&G system, columns for OOS, MMOS and SIL are not necessary and these columns are reduced on the dedicated formats. These formats shall be used for F&G system. 4, GENERAL CONCEPT 1) Cause & Effect Diagram shall be developed by UZ function. Plural function levels of ESD-1, 2, 3, EDP and PSS shall not be mixed in one UZ function. Other kind of function such as UY function in DCS shall be out of scope of Cause & Effect Diagram development. 2) As for reset function, dedicated reset switches shall be described in Cause & Effect Diagram. However, individual valve reset functions shall not be described. 3) In CAUSE section, trigger event or device which each effect is initiated by (e.g. HZ ‘switches, HSUZ switches, process conditions) shall be described. In PERMISSIVE CONDITION section, permissive conditions or other additional conditions to be satisfied for execution of the effect shall be described accordingly. Effects are ‘executed when all permissive conditions assigned in the same row are satisfied. 4) Generally, all trigger elements and conditions such as instruments and switches to be confirmed their status for activating function shall be described in CAUSE section : Nove Cause & Effect Diagram Procedure QCSISO/OO/IN/PCINA/D04 : B4E1 Page 5 of 16 5) 6) 7 8) or PERMISSIVE CONDITION section whether they will be connected with the UZ function by hard wiring or by software communication with/without passing through other function block. UZ function can activate lower level UZ function(s). (e.g. ESD-1 function activates ESD-2 function(s). ESD-2 function activates ESD-3 and/or PSS funetion(s)). In lower level UZ function diagram, associated adjacent higher level functions shall be described in CAUSE section. (Refer to Attachment-2 (2), (4), (6) ‘All effects activated by the UZ function shall be described in EFFECT section, Generally, final elements such as valves and equipments to be activated shall be described in EFFECT section whether they will be connected with UZ function by hard wiring or by software communication with/without passing through other function block. However, in case that lower levet UZ function is activated, only lower level UZ function shall be described in EFFECT section by way of exception. (Refer to Attachment-2 (1), (2)) When plural UZ functions activate a valve or equipment in parallel, actual output signal for the valve or equipment shall be controlled by the master UZ function which the youngest numbered UZ function shall be applied generally. In this case, the master UZ function number shall be provided in the row of CONNECTION in CRE Diagrams of other associated UZ functions (slave UZ functions) to show this parallel relation. And in the master UZ function C&E Diagram, description that the slave UZ functions activate the valve or equipment through the master UZ function shall be described with a proper note. (Refer to paragraph 6.3 2), Attachment-3) If regulatory controller function in DCS needs to be activated, this action shall be described in EFFECT section. As for symbol of this action, “C* or “O" is applied. Generally, “C" means controller mode is put into MANUAL and its output signal is set to fully close (maybe 0%). “O” means controller mode is put into MANUAL and its output signal is set to fully open (maybe 100%). “I*, which means interlock, shall not be used in this case because concrete action can not be understood. Additionally, "DCS" shall be described in the row of CONNECTION. (Refer fo Attachment-2 (2), (4), (5) and paragraph 6.3 2)) Strictly speaking, “fully open/close” mean maximum/minimum value to be available. In some case, control valves have to be remained open a little (e.g. about 5%) even though after shutdown. In this case, symbol of action on CE Diagram shall be also applied “C’, though this particular requirement shall be specified individually, : 9) Generally, generated alarms shall not be described as effect in C&E Diagrams for process unit. On the other hand, in C&E Diagrams for F&G system, all alarms on operator consoles and DCS shall be described. 10) Its difficult to understand the whole of UZ function hierarchy from only Cause & Effect Diagram. Hence ESD Diagram which shows the whole of UZ function hierarchy shall be developed at the same time and attached to the C&E Diagram document with a revision number for document control. 11) When function can not be described olearly by Cause & Effect Diagram such as burner light-off sequence, other proper diagram such as flowchart or narrative shall be provided for the details. TYPICAL DIAGRAM AND NOTES 5.1 ESD-1 (Emergency Train Shutdown) ‘Typical diagram is shown in Attachment-2 (1). 4) ESD-1 function will activate SD-2 UZ functions mainly, 2) Reset function, which will be initiated by reset switches on DCS display, shall be required. 5.2 ESD-2 (Emergency Unit Shutdown) Typical diagram is shown in Attachment-2 (2). 1) ESD-2 function can activate ESD-3 and/or PSS functions. 2) Reset function, which will be initiated by reset switches on DCS display, shall be required. 5.3. EDP (Emergency Depressurizing) Typical diagram is shown in Attachment-2 (3). 1) Confirmation requirements of associated shutdown UZ functions to be completed before the depressurizing shall be required as permissive conditions. In PERMISSIVE CONDITION section, only UZ function number to be confirmed shall be described as shown in Attachment-2 (3). And shutdown valves related EDP (shutdown valves located at boundary of zone) shall be listed on note column. 2) Operational Override Switches to bypass above confirmation requirements shall be required. 3) Reset switches are not required for EDP. Date: Nev: Cause & Effect Diagram Procedure QCS/5O/00/IN/PCINAVD04 48Feb.03,0506 | [Rev = B11 Page 8 of 16 5.4 ESD-3 (Emergency Equipment Shutdown) ‘Typical diagram is shown in Attachment-2 (4). 1) Reset switches are not required for ESD-3. §.5 PSS (Process Shutdown System) ‘Typical diagram is shown in Attachment-2 (6). 1) Areset switch shall be described in PSS functions which reset function is provided. 2) Operational Override Switches and Master Maintenance Override Switch groups shall be considered. §.6 _HIPPS (High Integrity Pressure Protection System) HIPPS can be regarded as a kind of PSS. Ex: QCS/52/02/PR/DR/PUO10 In case of the element such as gas detectors or smoke detectors in buildings, these are shown on layout drawings not on P&iDs. In such case, associated layout drawing number shall be described. DESCRIPTION ‘The function of the device shall be described. - For HZ switch or HSUZ switch, the location of the switch such as MCB, ITR-xx, Field, DCS, etc. shall be provided. In case of ITR, ITR number shall be provided. Ex.: Manual S/D switch (MCB) Manual EDP switch (ITR-71A) Manual S/D switch (Field) Manual Reset switch (DCS) HZ switches for ESD-1, 2 are push button type and equipped on auxiliary panel in MCB and ITR(s). © Master reset switches for ESD-1, 2 and PSS, HIPPS are on the display of Des. © HZ switches for ESD-3 are 2 positions type (ESD/NORMAL) and located in field. © HZ switches for EDP are 2 positions type (EDP/NORMAL) and equipped on auniliary panel in MCB and ITR(s). - For UZ function, function level shall be provided. Operational Override Switches are applicable to bypass the process condition only. (Refer to Attachment-2 (3), (5), (6)) 6) MASTER MAINTENANCE OVERRIDE SWITCH GROUP Maintenance Override Switches (MOSs), which will be latched type DCS software switches, will be provided for all 1001 shutdown process condition except air fin cooler vibration switches. And there will be a Master Maintenance Override Switch (MMOS) for a group of MOSs, located in the DCS. This MMOS will give the permissive on the individual MOS of the relevant group. In this column, this MMOS group number such as A, B, C, (Refer to Attachment-2 (5), (6)) . shail be provided. 7) SAFETY INTEGRITY LEVEL Associated Safety Integrity Level (SIL) of each cause / permissive condition shall be provided. 63 EFFECT Section ((C) in Attachment-1) 1) TAG NO., P&ID NO., DESCRIPTION Refer to paragraph 6.2. - As for ESDV, SDV, BDV, tag number of valve, not solenoid valve, shall be described. (Refer to Attachment-2 (2)) 2) CONNECTION ‘When the valve or equipment is activated through other UZ function or function out of IPS, or when UZ function makes a permissive condition for function out of IPS, this objective function number shall be provided in the row of FUNCTION and associated system name such as DCS, IPS shall be provided in the row of SYSTEM. If the objective function number has not been defined, only system name can be provided. In case the system is the dedicated one which belongs to package unit, the equipment number of package unit shall be provided in the row of SYSTEM. (Refer to Attachment -2 (2), (4), (5), (6), Attachment-3) NOTES ((D) in Attachment-1) If particular requirements have to be considered, they shall be described in NOTES area. PARTICUI \UIREMENT AS TO SHEET USAGE 1) Expansion / Reduction of PERMISSIVE CONDITION and EFFECT Number of columns for PERMISSIVE CONDITION and EFFECT are adjustable. Modify both numbers as required according to the required permissive conditions and effects in each UZ function. (Refer to Attachment-2 (6)) 2) Usage of plural sheets When one UZ function can not be described in one sheet, plural sheets need to be provided. in this case, followings shall be noted. Sheet number of the same UZ function shall be described, Row number of CAUSE and column number of PERMISSIVE CONDITION and EFFECT in all associated sheets shall be numbered in succession. Serial Number Three (3) digit numerical (001 through 999) shall be specified to identify the serial number of the loop of a particular instrument variable / function. = *000" shall not be applied. 8.2.6 Letter Suffix for Identical Equipment Unit This suffix shall normally be blank. In case plural same equipment are equipped in parallel, instruments for each equipment shall have the same Unit Code + Package Number + Serial Number and shall be distinguished by using this letter suffix. For this, suffix, same letter as suffix of each equipment name shall be applied. (such as A, B, ee) 8.2.7 Hyphen In case both of Letter Suffix for Function and Letter Suffix for Parallel Elements are blank, hyphen can be eliminated, (However in INtools database and associated document, hyphen will remain always.) 8.2.8 Letter Suffix for Function This suffix shall not be applied for UZ function and always eliminated. 8.2.9 Letter Suffix for Parallel Elements This suffix shall be normally biank but be reserved to identify the same elements in the ‘same loop or related function.
