Instructions for filling in this form:-


1. Read out this form carefully and prepare a draft form on the basis of details available to you or on the basis
of your memory.
2. In case, the details are not available with you, try to get the details from all the possible sources and even by
visiting the concerned offices of the Department, by approaching your good friends, colleagues, etc.
3. An individual RTI application, if required, is to be made to the office from where lastly retired for which all
necessary support will be provided by the group co.ordinators.
4. These details are required as this is a crucial stage of implementing the decision of Bombay High Court and
Board’s coming instructions and circulars on this subject matter of retrospective pay revision w.e.f.01.01.1996
on notional basis and w.e.f.21.04.2004 on actual basis(with benefits of arrears of pay and allowances and
retirement benefits including the enhanced pension)
5. For those who have not made any representation so far through Shri N.R. Parmar, group coordinator, they
are also required to fill in this form to the extent possible.
6. This form is not at all now required to send the filled in form to the group coordinator or to the Pensioners’
Association Office at Ahmedabad but has to keep with him and he/she will be contacted at the appropriate
time by personally contacting on their available mobile number or through Pensioners’ Association office.
7. The time given to fill in this form and keep ready with the applicant/member is of 15 days. Every
member/applicant will inform the group coordinator Shri N.R. Parmar(for applicants of I.T. Department) and
Shri B.M. Trivedi or Shri H.R. Shukla( for applicants of Central Excise & Customs Department through simple
whatsapp message stating that “My latest form is ready for submission”.
8. In case of any difficulty in filling this form, the applicant/member may contact Shri N.R.
Parmar(M.No.9586057493).
9. This form may also be forwarded to all the eligible known friends, colleagues who were holding the posts as
mentioned above at the relevant time and satisfying the following criteria:-
(i) Post held as Inspector of Income-tax in the Income-tax Department
(ii) (carrying pay scale of Rs.5500-175-9000 as per Vth Central Pay Commission recommendations
w.e.f.1.01.1996)
(iii) Post held as Income-tax Officer in the income-tax department.
(carrying pay scale of Rs.6500-200-10500 as per Vth Central Pay Commission
recommendations w.e.f.1.01.1996)
Post held as Superintendents/Appraisers of Central Excise Department(carrying pay scale of
Rs.6500-200-10500 as per Vth Central Pay Commission recommendations w.e.f.1.01.1996)
(iv) Post held as Superintendents/Appraisers of Customs Department (carrying pay scale of
Rs.6500-200-10500 as per Vth Central Pay Commission recommendations w.e.f.1.01.1996)
(v) Inspectors/Examiners/Preventive Officers of the Central Excise & Customs Department.
(carrying pay scale of Rs.5500-175-9000 as per Vth Central Pay Commission recommendations
w.e.f.1.01.1996)

• The applicants must have held these posts as on 01.01.1996.


• They must have been retired after 31.05.2004 in the above posts or higher posts on their promotions.
• They must have been promoted or directly recruited between the period 01.01.1996 to 20.04.2004 in the above-
mentioned posts as per the Circular dated 21.04.2004 which is under modification by the Board authorities.
