Sub: Medical Documents and Explanation Letter of Absence due to Medical Reasons

Dear Mr. Norman Lopez Digman II

Pax Christi!

I would like to inform you that attach in this letter are the copies of my medical
documents stating the reasons of my absence starting from April 1 to April 5, 2024.
As you may know that I have been sent to the emergency room and confined last April
2, 2024 at the estimated time of 5:20 A.M. of St. Luke’s Medical Center located at Bonifacio
Global City Taguig Philippines due to my chronic Upper G.I. pain that was caused by my
unknown or sub-prognosis from Maxicare Eton Centris (See Annex A for Documentation of
the Initial prognosis given by the Maxicare G.P.).
While I was given by the G.P. to have a bed rest of three (3) days, it was not followed
to the thoroughly for condition worsen.

I have disseminated the information that I was hospitalized to my teammates who


were contacted by my emergency s.p.a. informing and requesting them to provide this
information to you. I was then advise by my treating G.P. to have a consultation with a
specialist of gastroenterology to check the outlying issues in regards with my health.
Unfortunately the hospital Gastroenterologist has prior appointments with other patients,
that I was then advise to seek Out-Patient consultation with hospitals /clinics that have
them.
I was hospitalized from April 2 to April 5, 2024 at St. Luke’s Medical Center. Please
also note that medical documents such as but not limited to Medical Certificate, Prognosis,
Doctor’s Note will be available on April 9, 2024 based on the statement provided by the
record’s department since it is needed to be signed by the treating physician Dr. Azores,
Rouel Mateo M. & Dr. Rustia, Julius Carlo. The G.P. as St Luke’s Medical Center stated that
this can be the result of the long work hours before the lunch-break period based on the
history that I have. (See Annex A.1. for Emergency Situation that have happened in the work
place.) A initial diagnosis of peptic ulcer was given by the two G.P.’s. Employees work
condition are also the primary reasons of health related issues as cited in Scand J Work
Environ Health 2021.1

Before my discharge, on April 4th of 2024. I also have a video tele-consult from Dr.
Demapelis, Paolo Andrei giving the same diagnosis as my initial check-up with Maxicare Eton
Centris. (See Annex B for Teleconsult Diagnosis). I was advised to have an endoscopy to
further investigate the cause.

After my discharge this April 5th of 2024, I followed the G.P. advise to seek Out-Patient
consultation with a gastroenterologist from The Medical City Clinic Trinoma.
Dr. Uy, Darwin stated that multiple examinations should be done in order to point out
the specific prognosis of my case. (See Annex C for The Medical City Documentation and
Lists of Prognosis.) The examinations will be taken tomorrow April 6 th of 2024 to give insight
on what caused my illness.

As other documentation and evidences are attached in Annex D. (Discretion is


advised due to revolting pictures.

I do request for your kind understanding in regards with this matter. Hoping and
