Consent Form

This informed permission form is specifically intended for the instructor of

contemporary arts who assigned the students to conduct interviews solely for academic
purposes. We are senior high students enrolled in the 12th grade at the National University
located in Mall of Asia. The primary objective of our project is to conduct interviews with
artists from the local community to create our midterm output. The central concept of our
project is to demonstrate the positive impact of art on the local community, presented in the
form of a vlog.

The students of National University are seeking permission to conduct interviews

with local artists. Their involvement in this interview is optional. The local artists’ information
will be gathered and securely stored, with access limited to only the students in the group
and the teacher.

Prior to your agreement to participate, the students will respond to any inquiries on
the interview that is to be conducted.
