10/06/ Activity Intolerance D: Patient received awake, conscious and irritable. In


23; respiratory distress, O2 cannula was attached to her nose
11PM- at 4L per minute. Vital signs were BP: 110/60, T: 36.0 C,
7AM RR: 30, HR: 80, and O2 saturation: 88%. —--------------------

1 AM A: Placed patient in a moderate high back rest, Assisted


patient in passive range of motions, Instructed patient not
3 AM to do any unnecessary movement, Instructed patient to ask
for help if going to ambulate. Instructed patient to rest.

5 AM R: Patient didn’t display any signs of respiratory distress


anymore. O2 saturation increased to 99%. Patient was able
to rest. —-----------------------------------------------------------
