I acknowledge and accept that my son’s/daughter's/ward’s (the officer stated

named above) participation in the 1st Regional Executive Board Meeting (REBM) at
National University - Baliwag located in Complex, SM Baliwag, Doña Remedios
Trinidad Hwy, Baliwag, Bulacan is entirely voluntary, and all risk is voluntarily assured by
my son/daughter/ward and me.

I understand that organization rules and regulations will be in effect. I have also
ensured that my son/daughter/ward understands that it is important for his/her safety,
and for the safety of the group, that all rules and instructions given by the organization
supervisors are obeyed.

In consideration of your agreeing to take my son/daughter/ward on the

abovementioned activity, I hereby agree not to hold the organization and any
individual involved in the organization liable for any expense, loss, personal injury, or
accident to my son/daughter/ward which is not the result of any negligent act or
willful default of any officer of the organization. I will not bring any suit or assert any
claim against NFJPIA – Region 3 or the organization’s supervisors as a result of any
action taken.
