ROLES AND RESPONSIBILITIES OF
TEAM LEADER: • Assist in the selection of team
members (if relevant and applicable
• Lead the assessment team (represents
the team in communication process)
• Ensures all aspects in assessment
activities are adequately covered
• Provide direction and guidance to the
team members
• Preventing and resolving conflicts
• Making decisions in coordination with
PAB , if necessary
• Drafting, coordinating and submitting
the assessment report in such a way
that it allows the accreditation body
and the assessed organization to take
actions and decisions
DURING ON-SITE • Facilitates the team briefing, opening
meeting, team meeting and closing
ASSESSMENTS: meeting
• ensure that a balanced assessment is
conducted

questions regrading PAB policies and
procedures

AFTER ASSESSMENTS:

to PAB within thirty (30) calendar days
• Maintain confidentiality
• Evaluate the assessment team

GROUNDS FOR SUSPENSION:
WORKSHOP: List all the cases/scenario where an
assessment can be suspended/discontinued

1. When an accredited CAB signifies that it is


temporarily unable to comply with the accreditation
requirements.
2. When an assessment reveals that an accredited CAB
has failed to comply with the requirements for
accreditation and corrective action cannot be
taken/submitted to achieve full compliance.
3. Inappropriate use of PAB accreditation symbol (Refer
to LA/SR11 PAB Requirements for the Use of PAB
Laboratory Accreditation and Inspection Body
Accreditation Symbols).
4. When the CAB fails to pay its Billing Responsibilities
two (2) months from its due date, this includes
Assessment Fees, Annual Fees, and Accreditation
Fees.
CRITERIA FOR LIFTING
SUSPENSION:

1.Suspended CABs shall apply for a


special assessment within six (6)
months and be recommended for
continued accreditation.
2.Suspended CABs shall submit within
six (6) months the corrective action
taken on the misuse of PAB
accreditation symbol.
3.Payments shall be settled within six
(6) months from the date of
suspension.
