Critical Incident Analysis Clinical Week 6th

Description:
I visited emergency department as assign my preceptor Ms. Sarah William. A head nurse is
working there as a nurse manager in emergency medical wards and there were three staff nurses
working. Nurse Manager has great hold on the ward and the employees. All nurses were
managing the patients very carefully. They were giving critical care to the patients, administering
the medication to the patients and maintain all the records.
while visiting, I observed an incident, One RN was inserting nasogastric tube to a patient in
upright position without using lubricant patient seems very uncomfortable during insertion of
NG tube this procedure seems very painful for the patient.

Feelings:
I did not feel so good to see few flunks first they were over burden of patients and only two
doctors and three nurses were managing the patients. secondly the procedure of insertion of
nasogastric tube was not properly done in decent manners it become very painful for the patient
passing nasogastric tube without any lubricant and patient seems discomfortable while the
procedure

Evaluation:
I saw in the emergency the nasogastric tube insertion procedure was no properly done Nurse
should use lubricant while passing nasogastric tube it will be less painful for the patient and less
risk of injury during insertion of the tube. Without using lubricant, it can be an injury inside
nasal mucus and interior wall of throat can be damaged, because without lubricant tube can
damage internal side of nasal because of resistance.
Because of overburden of patient’s staff is not able to provide the quality care to the patients.
All procedures should be done in proper way.

Analysis:
In the emergency department, I analyzed those Aseptic procedures are not performing well by
the staff nurses and insertion of nasogastric tube passed without using lubricant.

Conclusion:
During the nasogastric tube insertion lubricant should be used it is necessary to use it from the
prevention of any injury in side nasal mucus and nasal concha. The staff nurses need proper
training of these type of procedures.
