FORM FILLING INSTRUCTIONS

 All applications must be filled under TORONTO
JURISDICTION.
 All Personal Particulars must be filled exactly as
per Indian Passport.
 All names on the application form must be mentioned
exactly as per Indian Passport.
 It is mandatory to mention both parents’ name on the
Application Form.
 Foreign National Acquisition date must be as per
Canadian Oath / Citizenship certificate.
 Applicants need to Mention Canadian Address on
Application Form.
 Self-declaration on plain paper is required if:
• If Citizenship card does not have complete date
of Acquisition of Foreign Nationality
 For Minor Applicants, Parents SHOULD NOT sign the
application form. Applicant’s Signature / Thumb
Impression are required.
