Incident

was slipping on Water Treatment Plant.

Sergio Massimbe was found slipping on Water Treatment Plant on lunch time

Consequences:
Although it was on lunch time and hi told me that was sick, it is not safe to slip in the Plant.

Corrective action

Verbal owning was given to him and hi promised not do it again.
