AFFIDAVIT

I, CARLOS C. TAN, JR., of legal age, married, Filipino citizen, and a resident of
Camangaan East, Moncada, Tarlac, after having been duly sworn to in accordance
with law, do hereby depose and say, to wit:

1. That I am the registered owner of a parcel of land located at Camangaan,

Moncada, Tarlac and covered by Transfer Certificate of Title No. 043-
2020002362;

2. That on the title issued by the Registry of Deeds of the Province of Tarlac, the
technical description of the aforementioned parcel of land, which read as
follows:

“A PARCEL OF LAND (LOT 1-3-4 OF THE SUBDIVISION PLAN

PSD-03-165675, BEING A PORTION OF LOT 1-B PSD—03-
164816, LRC REC. NO.);”

3. That the technical description appearing on the title was “LOT 1-3-4” instead
of “LOT 1-B-4”, which is the true and correct description of the subject parcel
of land;

4. That said discrepancy in the TCT was an error done inadvertently;

5. That I am executing this affidavit to attest to the truth of the foregoing facts,
and for the correction thereof in the TCT by the Registry of Deeds.
