In connection with the proposal of training for Certification of Legal Auditor (CLA), the company already

convey that for payment by company the related employee should agree the condition that restrict
resignation for period 3 (three) year.

Hereby, I would like to request consideration to President Director to adjust the condition detail as
follow:

1. The benefit of training which involved CLA Training Certificate [Original] and Certified Legal Auditor
Card will be retained by the Company under supervision of GA&HR Department. Related employee
prohibited to use or take out the certificate and auditor card if resign within period 3 (three) years.
2. If the related employee resign within period 3 (three) year after training and demand to take the
certificate and auditor card, related employee should return training costs in to the company and will
be confirmed on employee exit clearance by GA&HR Department & Accounting Department.
