My name is Mark, I grew up in Guilford, and by the age of 14 I was an addict
I was adopted, I grew up in Guilford and by the age of 14, I was ba ling addic on. I started out smoking pot, then bingeing on alcohol, and eventually added cocaine. For the next 30 years, my only purpose in life was to drink and do drugs. You may or may not be surprised to learn that for most of that period, I was technically homeless. Id actually worked out a pre y good system to keep my self o the streets. I would go down to the hospital in New Haven, tell them I was suicidal and theyd check me in for a few days, and some mes a few weeks. That way I had a warm place to sleep and people to take care of me. Being suicidal wasnt a

complete lie. I do struggle with major depression and have a empted suicide in the past in fact, in 1996 I almost succeeded. I purposely overdosed on cocaine and pills (downers mostly), I wound up in a coma, had a stroke and spent the next three years in a wheelchair. Somehow though, even that didnt stop me from ge ng high. That didnt happen un l much later, in fact not un l October 4, 2010. I was back in the inpa ent unit in New Haven, doing my thing, planning to get drunk as soon as I was released, and my doctor came in to speak with me. He basically told me that the jig was up, that I couldnt con nue checking myself into the hospital, that I needed to get help once and for all and start taking care of myself, or start living on the streets more permanently. He wanted me to try Antabuse, (used for the treatment of alcohol abuse) and I said, No. Being the good doctor that he was, he walked away, but his words resonated with me over the night, and the next morning something in me changed. I wanted to try to get sober and I asked him for help. I started the Antabuse that morning, and I havent touched a drink or illegal drug since. I was discharged from the hospital and admi ed to the HARP program at Harbor Health. I was immediately enrolled in an intensive outpa ent program (IOP) for people just like me who were ba ling to stay sober, and struggling with mental illness. I spent ve days a week here (at Harbor Health) three in the clinic, and

even after you graduate, youre not forgotten.

two up at OPTIONS where I worked with my case manager, housing specialist, and par cipated in recovery support groups. I graduated from the IOP program, but I am not alone. I can pick up the phone any day of the week and someone at Harbor Health is available to me. Thats what I truly cherish, even a er you graduate, youre not forgo en. I would be lost today without Harbor Health. They dont just handle addic on, they cover all aspects, mental health, nancial issues, housing, family issues. They even brought my Mom in for a couple of my sessions to help us get back on track. Harbor Health helped me rent a co age in Madison so now I have a place to call home. They help manage my nances, because money is a trigger for me. It feels good to pay my bills. I used to look down when I passed a mirror, but now I look up, when I look in the mirror I feel proud and its an amazing feeling. Im not saying that recovery is easy. I have a sponsor, I go to mee ngs and my rou ne is very important to me. Im looking forward to my one-year anniversary this October, and I think down the road I may go into recoveryrelated work. Out of all of this Ive learned that no situa on, no illness and no disease cant be dealt with in a posi ve way, on a daily basis. Its okay to ask for help, and once you do, youll feel proud too. 4

Dear friends, Looking back on the last year, we are so grateful for all that youve helped us provide. Thanks to your generosity and support, more than 1,500 of your friends and neighbors have access to cri cal mental health, substance abuse and community support services, right here in their local communi es. We all know that the landscape of healthcare is ever changing, and we here at Harbor Health Services do all that we can to evolve as the system advances and develops. There are a lot of exci ng endeavors on the horizon for the next year, most importantly, our upcoming merger with Birmingham Group Health Services in Ansonia. The merger provides a fantas c opportunity for us to expand and enrich our services, provide treatment and support for more people and extend the reach and impact of our mission. Over the next 18 months we will integrate programs and services throughout our 40 town service area and will grow to treat more than 13,000 individuals and families. We thank you for your con nued support during this important me and look forward to working with you as we expand our horizons. 5

Programs & Services


Clinical Services:
HHSI provides a wide range of comprehensive psychiatric and substance abuse treatments. Treatment rou nely includes assessment, diagnosis, prescrip on and administra on of psychotropic medica ons, group and individual therapy, psycho-educa on, and reviews of medical health history. Counseling services in Spanish are made available through CommuniCare and the CT La no Behavioral Health System. Overall last year 1,572 individuals received 19,048 clinical services at HHSI.

Crisis Services:

HHSI, through CommuniCare, provides both on-site and mobile crisis services, 24hrs/7 days per week, for those clients who experience severe or cri cal mental health episodes. Walk-in crisis services are available at our outpa ent clinic, Mon. - Fri., 8:30 am - 5 pm. Mobile crisis services are also available during these hours for persons who are homebound. A er hours, weekend and holiday crisis services are provided through the South Central Crisis Service (SCCS). Over the last year, HHSI provided 445 crisis services to 209 individuals.

Community Support Services:

The Community Support Program is designed to assist adults with severe and persistent mental illness and/or

substance use disorders in achieving and maintaining rehabilita ve and recovery goals. Services address the mental health/substance abuse treatment needs of individuals as well as provide nancial, social, educa onal, voca onal, residen al, jail diversion and other treatment and support needs of individuals. In the last year 1,343 individuals received 8,255 community support services.

Residen al services provide clients with 24/7 on-site support. The Harbor Apartment and Respite Program (HARP) is an agency owned/managed apartment complex that oers services in a cluster apartment model to individuals who can benet from sta supervision 24 hours a day while living in their own apartment. Harbor House is an eight bed, con nually staed adult psychiatric community residence that provides a safe, structured, and suppor ve environment in the community in which residents with serious and persistent mental illness are assisted in achieving their highest degree of independent func oning and recovery. In 2010, HHSI provided residen al services for 67 individuals, who bene ed from 23,345 services.

Residential Services:

The tobacco cessa on program began in 2010 thanks to a grant awarded to CommuniCare by the CT Department of Public Health. The program includes tobacco cessa on counseling and nico ne replacement therapies/medicine for nico ne dependent individuals. The program links clients with Nico ne Anonymous

and the CT Quit-Line to help support their recovery. To date, 176 HHSI clients have enrolled in tobacco T Quit-Line cessa on services, of those enrolled, 95 percent reported a history of failed quit a empts, and 62 percent of those who completed the program report either reducing their tobacco use, or complete cessa on.

Health Management Strategies for Recovery:


Our on-site primary care program, Health Management Strategies for Recovery (HMSR) is funded by a grant from the Substance Abuse and Mental Health Services Administra on, and is a CommuniCare ini a ve. HMSR has been increasingly successful in providing services to our community members. 188 clients are currently enrolled in the HMSR Program, and are assessed by the nurse care manager on an average of every two to three months. Over the last year, the nurse care manager provided 278 assessments. 104 clients, who had no access to a primary care doctor, are now receiving on-site primary care services through our partnership with the Cornell Sco Hill Health Center. 10

I A tC In August Congresswoman R L D L Rosa L. DeLauro (C (CT-3) visited CommuniCare to par cipate in a ribbon cu ng ceremony to celebrate our $2 million federal primary care grant. The funds are being used to integrate primary care into each CommuniCare partner agency clinic: Harbor Health Services in Branford, Birmingham Group Health Services, of Ansonia, and Bridges, and A Community Support System, of Milford. On October 1, we began providing oversight and support for the Town of North
Haven counseling services.

milestone. The cita on stated, You have improved the quality of life for many people in the Shoreline Community. You have demonstrated a remarkable commitment to the highest quality of behavioral healthcare while working with individuals through all phases of recovery. You are a ne example of collabora on with both individuals and families in ways that help them achieve their full poten al.

Thanks to a grant from Alcoa Howmet of Branford, we launched our new website
in January 2011. raised $5,000 and drew a crowd of 175 women to Terminal 110 in New Haven on March 18. Guests enjoyed a fabulous fashion show by Ella, where She shops of Guilford, hair by Echo Salon, makeup by Jennie Fresa Beauty Library, and fabulous jewelry by Amy Leiner. Terminal 110 served delicious food & drinks. Guests also enjoyed bou que shopping, raes and a silent auc on. 12

Our second annual Girls Night Out Fashion Show Fundraiser was a great success, it

Our 5th Annual Road to Recovery Gala and Auc on was held June 3 at Amarantes

Sea Cli and was a tremendous success raising more than $20,000 to support our programs and services. The Gala also gave us the opportunity to recognize Charlo e Ma ei for her hard work and volunteerism, and State Representa ve Patricia M. Widlitz as our Community Partner for her years of con nuous support and eort on behalf of our clients and our organiza on.
