1. Approved Baguio City examinee with NOA but no name in the qualified list;
2. With discrepancies in the name (Family Name, Given Name and Middle Name);
3. With discrepancies in the Date of Birth;
4. Incorrect/Wrong name of school;
5. Pregnant at the time of examination, with Disabilities and/or with Medical Condition

For first-time examinees not included in the posted list, please request from your School Registrar to
send an electronic copy of the Notarized List of Graduates at car.application@prc.gov.ph

Room Assignments will be posted three days before the first day of examination at www.prc.gov.ph
