Assessment
Subjective: Ang daming beses nya tumae everyday bago sya maadmit dito. as verbalized by the mother. Objective: -Increased bowel sounds - Yellow-watery stool - Abdominal Pain -Pain scale of 5 out of 10

Nursing Diagnosis
Diarhhea related to presence of toxins as manifested by frequent elimination of loosewatery stool.

Inference
Diarrhea is the condition of having three or more loose or liquid bowel movements per day. Toxins that attack GI Track stimulates Digestive System increase bowl movement rate in response to toxins secreted by pathogens. The loss of fluids through diarrhea can cause dehydration and electrolyte imbalances.

Goal
Short Term: After 8 hours of nursing interventions, the Client will: Score Pain from 5-2 using Pain scale. Lessen peristaltic motility Long Term: After 1 week of nursing interventions, the client will: Reestablish and maintain normal pattern of bowel functioning. Abdominal Pain would diminish. Digestive System of client would return to normal state. Have more normal stool consistency.

Nursing Intervention
Independent Observe and record frequency, characteristic, amount, time of day, and precipitating factors . Monitor intake and output.

Rationale
Helps differentiate individual disease and assesses severity of episodes. To monitor for excessive fluid loss of client. Antibiotics often cause changes in bowel habits, especially in children. Avoiding intestinal irritants promote intestinal rest. To restore lost fluids and electrolytes. To decrease gastrointestinal motility.

Evaluation
Short Term: After 8 hours of nursing interventions, the client was: Able to score pain 2 out of 10 from 5 out of 10. Peristaltic motility of client was lessen Long Term: After 1 week of nursing interventions, the client: Reestablished and maintained normal pattern of bowel functioning. Pain was diminished Digestive System returned to normal Had more normal stool consistency.

Review medications, noting side effects, particularly antibiotics.

Encourage intake of non-irritating liquids. Dependent Administer medications for Rehydration Administer antidiarrheal
