NOTARIZATION REQUIREMENTS

1. Check and review the correctness of the following before printing:


 Names
 Address
 Lot no.
 TCT/OCT no.
 Tax Dec no.
 Location of the property
(The lawyer will not counter-sign documents already submitted for archive)

2. Attach three (3) photocopies/photos of current valid ID of all signatories with


signatures, such as but not limited to:
o Passport
o Driver’s license
o Professional Regulations Commission ID
o National Bureau of Investigation clearance
o Police clearance
o Postal ID
o Voter’s ID
o Barangay Certification
o Government Service and Insurance System (GSIS) e-card
o Social Security System (SSS) card
o Philhealth card
o Senior Citizen Card
o Overseas Workers Welfare Administration (OWWA) ID
o OFW ID
o Seaman’s book
o Alien certificate of registration/immigrant certificate of registration
o Government office ID
o Certification from the National Council for the Welfare of Disable Persons
(NCWDP)
o DSWD Certification

Attach specimen signature of signatories beside their IDs and make sure the signature
in the ID matches the signature in the documents.

3. Attach supporting documents for the following:


 Affidavit of ownership
 EJP with SPA

4. Practice “writing the name first before signing” and as much as possible, use one/same
type/kind of ballpen in writing and signing.

5. Insure document COMPLETENESS. Don’t leave blanks.
