Accidents are caused by chemical, mechanical, civil, electrical, or other process failures due to accident, negligence or incompetence, which may spill over to the areas across causing damage to life and property. These may originate in:  Manufacturing and formulation installations including during commissioning and process operations; maintenance and disposal of Oil.  Material handling and storage in manufacturing facilities, and isolated storages; warehouses and godowns including tank farms in ports and docks and fuel depots.  Transportation of oil (road, rail, air, water, and pipelines). Major Threats: Fire Explosion Toxic Release

Combination of all the three threats as well

On 7/6/1988, a series of explosions and fires destroyed the Piper Alpha oil platform 165 platform workers and 2 emergency responders were killed
,

61 workers survived by jumping into the North Sea Oil Rig Explosion
