Problem Presented: Above named client sought assistance from the office because of their Daughter Stephanie Aguilab, who was born with a cleft lip and a cleft palate, is scheduled for an operation this year. However, due to lack of financial resources the family could not afford the operation and hospitalization fee. The bunkhouse they were occupying was burned last November 12, 2007 and the money that was intended for the operation was utilized for it.

IV.

Findings and Background information: Stephanie is now 2 years old and was brought to Baguio General Hospital for further evaluation. His parents want her to be operated this vacation together with their son (Jan Dynver), to save time and effort.

Problem Presented: Above named client sought assistance from the office because of her right eye had a blurry vision and ached after doing some of the household chores. She had an operation on the other eye, left eye, because of cataract. She relayed a story that one time when she was weeding the ground, she accidentally hit her eye with the weeds she was trying to pull the weeds. Due to eye pain at blurring of vision, client went to the clinic to sought consult and was referred to the Baguio General Hospital for an eye for further evaluation. She was diagnosed of Left eye cataract and was due for an operation.

VIII. Findings and Background information: Beatrice is now having a hard time seeing things with her right eye especially in the afternoon. She claims that her right eye is painful mostly in the afternoon and states that has taken no medication for her eye.
