IDENTIFY HAZARD CONTROLS For each hazard checked above, list the corresponding hazard number and explain how employees will be protected from injury: engineering or work practice controls, personal protective equipment (PPE). Control- Follow label when mixing chemicals, use PPE per label, use in well ventilated area, avoid wind. Control- Wear clothing appropriate for conditions, watch for signs of hypothermia. Control- Use proper eye and respiratory protection. Control- Use good body mechanics when lifting or moving heavy objects. Control- Avoid contact with downed power lines or trees leaning against power lines.

Hazard No. 11 Control- Use sun protection, stay properly hydrated, take breaks out of the sun. Hazard No. 14 Control- Use correct tool for job, follow manufacturers guidelines for PPE including gloves. Hazard No. 17 Control- Take breaks to avoid hand fatigue, use anti-vibration gloves. Hazard No. 18 Control- Only work in adequate light. Wear reflective safety vest or other high visibility gear. Hazard No. 19 Control- Wear hearing protection. Safe Operation Procedure (SOP) and PPE required for job: Follow manufacturers guidelines for operating all equipment. Follow appropriate OPRD safety module guidelines for each task. Wear PPE as directed by operation manual or safety module.
