This is to confirm that the above named is an old patient of our hospital, she
presented us with Gastric Ulcer, High Fever and Typhoid.

She has once been diagnosed but still complain of the same symptoms, she will
subsequently book for another appointment but had to be postponed, due to Clarion
call.

She's also placed on the following medication; Boscopam, Gecrol,


Theophylline, Acetaminophen, (Tylenol, ibuprofen, (Advil, Motrin
IB)).

On medical ground, i will request her relocation back to EDO STATE


for proper medical treatment from his health care providers to ensure
uneventful service year.
