    MEDICAL REPORT IN RESPECT OF ISHAKU NGWE CHRISTOPHER,
    AGE: 28 YEARS

    The above named presented in the General Out-patient Clinic of this


    hospital with complaints of severe headache, chest pain, abnormal heart beat,
    dizziness, allergic to cold and fatigue.

    Investigation and diagnoses confirmed that he has suffering from


    PNEUMONIA and MYOCARDITIS and has been under the management of
    the cardiology unit and close supervision with his Pulmonologist in the hospital.

    In view of the above we strongly advise that he be redeployed to Bauchi


    State, this is to enable him access to his Pulmonologists and cardiologist forth
    nightly as well as very close monitoring from his family and relatives to avoid
    an emergency situations.
