ACTIVITY-EXERCISE PATTERN The purpose of assessing the client s activity-exercise pattern is to determine the client s activities of daily living, including routines of exercise, leisure and recreation. This includes activities necessary for personal hygiene, cooking, shopping, eating, maintaining the home and working. An assessment is made of any factors that affect or interfere with the client s routine activities of daily living. Activities are evaluated in reference to the client s perception of their significance in his or her life. Subjective Data: Guideline Question Activities of Daily Living  Describe your activities on a normal day (including hygiene activities, cooking activities, shopping activities, eating activities, house and yard activities, other self-care activities)  How satisfied are you with these activities?  Do you have difficulty with any of these self-care activities? Explain.  Does anyone help you with these activities? How?  Do you use any special devices to help you with your activities?  Does your current physical health affect any of these activities (i.e. dyspnea, shortness of breath, palpitations, chest pains, pain, stiff, weakness)? Explain. Leisure Activities     Describe the leisure activities you enjoy Has your health affected your ability to enjoy your leisure? Explain Do you have time for leisure activities? Describe any hobbies you have.

Exercise Routine  Describe those activities that you believe give you exercise.  How often are you able to do this type of exercise?  Has your health interfered with your exercise routine?

Occupational Activities     Describe what you do to make a living. How satisfied are you with this job? Do you believe it has affected your health? If yes, how? How has your health affected your ability to work?

Limitations  Do they have any medical or physical problems that interfere with participation in physical activities, personal care, home maintenance, or other activities?  Are precautions taken for those who have a high degree of immobility, such as turning, moving and skin care? Driving assessment  Do they currently drive?  Do they have an active driver s license?  If so, have they had any accidents in the last 5 years?  If any accident occurred, obtain a detailed history regarding the accident(s). Was alcohol involved? Did it occur at night? Have they ever received minor traffic violations or had their license revoked? Have they had a driving test preformed recently?  Have they made adjustments to their driving (avoid driving at night, only drive when traffic is light, only drive to familiar locations)?  Do they have a health condition that may affect their ability to drive, such as osteoarthritis (difficulty with movement because of pain), seizure disorder or other neurological disorder(s), diabetes (possible hypoglycemia), or sleep apnea?  Are medications a potential problem, such as anti-hypertensives, hypoglycemic agents, pain medication, or other medications that cause drowsiness?  Has anyone ever suggested that they should no longer drive? Was it a health care professional?  Do they feel safe driving?  Are they the primary driver in the family?  Do they wear their seat belt while in the care?  Who else drives in the family? How are transportation needs managed in the family?

Social/recreational activities  Do they have outside interests or hobbies that provide enjoyment and diversion?  Have they made recent changes in diversional activities?  Do health problems limit participation in hobbies or recreational activities?  If they have given up activities, have they identified other areas of interest to pursue?
