NEONATAL RESUSCITATION ALGORITHM

Antenatal counseling.
Team briefing.
Equipment check.
BIRTH

Term gestation?
Good tone? YES Stay with mother for initial steps, routine care, ongoing evaluation.
Breathing or crying?

NO
1 MINUTE

Warm, dry, stimulate, position airway, suction if needed.

Apnea or gasping?
HR <100 bpm? NO Labored breathing or persistent cyanosis?

YES YES

PPV. Position airway, suction if needed.


Pulse oximeter. Pulse oximeter. O2 if needed.
Consider cardiac monitor. Consider CPAP.

HR <100 bpm? NO Post-resuscitation care.


Team debriefing.
YES

Ensure adequate ventilation.


Consider ETT or laryngeal mask.
Cardiac monitor.
Target O2 Saturation Table
NO 1 min 60% - 65%
HR <60 bpm?
2 min 65% - 70%
YES
3 min 70% - 75%
ETT or laryngeal mask. 4 min 75% - 80%
Chest compressions. 5 min 80% - 85%
Coordinate with PPV- 100% O2.
UVC. 10 min 85% - 95%
NO Initial O2 concentration for PPV
HR <60bpm?
YES
~ 35 weeks’ GA 21% O2
< 35 weeks’ GA 21% - 30% O2
IV epinephrine every 3-5 minutes.
If HR remains <60 bpm,
• Consider hypovolemia.
• Consider pneumothorax.
