In t he event of i nj ury fol l owi ng an acci dent : - in port , noti f y your owner or m a nager and t he P &I correspondent . At sea, noti f y your owner or m ana ger, obt ai n radi o m edi cal advice. As well as com pl et ing t he accident report , wri t e a det ai l ed descripti on of what occurred ( these not es wil l hel p to refresh your m em or y duri ng t he subsequent int ervi ew wi th a l awyer). As k wit nesses t o writ e a det ail ed descr ipt ion of what t hey saw or heard ( you wi ll need a speci al form for this whi ch is supp li ed by your owner or m anager). If s hips equipm ent or t he shi ps st ruct ure was involved in the inj ur y, ex ami ne the equi pment , t ake photographs of the pl ace where t he acci dent is said t o have occurred ( and record t he t im e and the dat e of phot ographs). W hil e i t is appreci ated that your S afet y Managem ent S yst em has a procedure coveri ng acci dents and i nj uri es on board, requirem ent for the Mast er or Operator t o report Acci dents and Maj or Inj uri es b y t he qui ckest m eans poss ible and as soon as possi bl e aft er the occurrence to t he Marine Acci dent Inves t i gat ion Branch. S eri ous Inj uri es shoul d be report ed wit hi n 14 days and

whil st t here i s no requirem ent to report a haz ardous Inci dent it i s recomm ended that a report i s sent. The definiti ons of Acci dents and Maj or Inj uri es and S eri ous Inj uri es are contained i n MGN 289. Acci dents shoul d be report ed t o t he Mari ne Accident Investigat i on b ranch on form IR F and you should have copi es of t hi s form on board.
